How to Register

To register with our practice please follow the link below to register online

Please ensure you answer all questions fully and use your NHS number if you have it.

OR you can download the forms below and after completion please submit the FULLY completed forms to [email protected]

  • GMS1 FORM
  • OFP ADULT REG FORM
  • OFP CHILD REG FORM V2

Non-urgent advice:

Acceptable Identification: tenancy agreement, mortgage agreement, bank statement, driving license, passport, birth certificate etc. If you do not have proof of address please discuss with our reception team.  This will not prevent your registration.

Until you have submitted your ID you will not be registered at the practice.

Registration Medical

Once registered, , if you are over 40 make an appointment with our health care assistant for a new patient health check.

We invite all our new patients to see one of our practice nurses or our health care assistant. This appointment enables us to collect medical information before your medical records arrive. It is also the start of screening for other medical problems.

If you are on regular medication please ensure you have sufficient from your previous practice before registering as we may not be able to issue a repeat prescription immediately.

Temporary Residents

If you are not registered with a doctor but need to see one, you can receive emergency treatment for 14 days. After 14 days you will need to register as a temporary or permanent patient.

To register as a temporary resident you will need to complete a Temporary Resident Form. Please return the completed form to the surgery.

You can register as a temporary resident with a GP in England if you are in the area for longer than 24 hours but less than three months.
