How to Register
To register with our practice please follow the link below to download and complete the online registration forms below.
THE GMS1 FORM MUST BE SIGNED BY THE PATIENT OTHERWISE WE WILL NOT BE ABLE TO REGISTER YOU
Please complete FULLY before submitting via email to mail.ofp@nhs.net during the Covid 19 crisis with identification (ID) and proof of address. Please include a passport style photo as additional ID during this crisis, of each person registering.
Acceptable Identification; tenancy agreement, mortgage agreement, bank statement, driving licence, passport, birth certificate etc . If you do not have proof of address please discuss with our reception team. This will not prevent your registration.
Until you have submitted your ID you will not be registered at the practice.
Registration Medical
Once registered, , if you are over 40 make an appointment with our health care assistant for a new patient health check.
We invite all our new patients to see one of our practice nurses or our health care assistant. This appointment enables us to collect medical information before your medical records arrive. It is also the start of screening for other medical problems.
If you are on regular medication please ensure you have sufficient from your previous practice before registering as we may not be able to issue a repeat prescription immediately.
